Population Health Management: How can it help you with planning SMRs?

Population Health Management aims to improve local population health by data driven planning and delivery of proactive care to achieve maximum impact. It includes segmentation, stratification and impactabilty modelling to identify local ‘at risk’ cohorts – and, in turn, designing and targeting interventions to prevent ill-health and to improve care and support for people with ongoing health conditions and reducing unwarranted variations in outcomes.

Dr Wasim Baqir, Senior Pharmacist, Pharmacy Integration Programme Primary Care, NHS England and Improvement told us about it and how as medicines experts and PCN leaders we need to make sure we are utilising what it can do and improving its use for our high risk populations.
