We heard about two great MO projects that should make you stop, think and refocus.

Rachel Howatson, Senior CVD Pharmacist for SE London, talked about a quality and safety audit of direct oral anticoagulant (DOAC) prescribing and monitoring. She covered its impact on patient safety outcomes alongside remodelling of SEL interface pathways for DOAC patients, local DOAC prescribing guidance for the ICS and DOAC monitoring process development.

Caryn Chan, Medicines Optimisation Pharmacist NECS, told us about a novel risk assessment tool CATALYST which identifies patients with an inaccurate label of penicillin allergy for de-labelling without direct oral penicillin challenge in primary care.

Taming the structured medication review! A multidisciplinary approach to polypharmacy

We welcomed Professor Emma Baker, Professor/Consultant in Clinical Pharmacology and Consultant Physician St George’s Hospital, London, specialising in Clinical Pharmacology and Therapeutics.

She leads a polypharmacy specialist service in South West London, with the aim of enabling better medication reviews across the whole integrated care system.

Emma talked to us about how she and her team approached the challenges of structured medication reviews, including taking a risk:benefit approach to deprescribing, having complex conversations with patients and when to ask for help.

We welcomed Jamie Hayes, Director of the Welsh Medicines Resource Centre and Executive Coach.

Not enough time to do thorough job? Are you seduced by your clinical workload? Do you treat all demand as work?

These are traps that can be overwhelming, stealing your time and your energy. By taking systems view and learning how to listen to, identify and analyse the demand faced by you and your teams, there is an opportunity for you to be more effective.

By identifying value demand and removing failure demand you and your teams can concentrate on what matters, making work more rewarding, energising and impactful. The importance of focusing our time, effort and energy on providing the most professional value is crucial during these times of high staff vacancies and low staff morale.

For this session Jamie explored the application of systems thinking, organisational health, and the efficiency-thoroughness trade off.

Managing Analgesic Reviews in people without capacity / NICE guidance on dependence forming medicines (DFM)

Hypertension management across your locality

Alison Warren Consultant Pharmacist Cardiology at University Hospitals Sussex (Brighton) and Sussex Clinical Commissioning Groups brought us up to date about the latest NICE update on Hypertension and made us think how to work collaboratively to help reduce cardiovascular burdens across our local populations.

Alison was the pharmacist representative on the NICE Hypertension guideline committee so really knew what she was talking about.

Debi Bhattacharya, Primary Care Pharmacist & Professor of Behavioural Medicine at the University of Leicester, talked to us about the evidence base for, and the practical aspects of, Behavioural Science and how it can help us implement/ improve deprescribing for with our practice population.

It is an incredibly fascinating subject that all Pharmacy Professionals need to understand and hopefully utilise to improve Medicines Optimisation Stewardship.

Hetal Dhruve, Pharmacist and Clinical Research Fellow at Guy’s & St Thomas’ NHS Foundation Trust Asthma Centre returned to update us on Asthma care, in particular the challenging IIF respiratory PCN targets that necessitate a team approach to improving asthma control and changing people from MDIs, where clinically appropriate, to reduce Carbon emissions.

Population Health Management: How can it help you with planning SMRs?

Population Health Management aims to improve local population health by data driven planning and delivery of proactive care to achieve maximum impact. It includes segmentation, stratification and impactabilty modelling to identify local ‘at risk’ cohorts – and, in turn, designing and targeting interventions to prevent ill-health and to improve care and support for people with ongoing health conditions and reducing unwarranted variations in outcomes.

Dr Wasim Baqir, Senior Pharmacist, Pharmacy Integration Programme Primary Care, NHS England and Improvement told us about it and how as medicines experts and PCN leaders we need to make sure we are utilising what it can do and improving its use for our high risk populations.
