Following the 2021 update to the NICE CKD guidance we were joined by Natasha Moore, Advanced Renal Pharmacist , Guy’s and St Thomas’ Hospital. She helped us deal with the most important aspects of living with CKD when we see patients for SMRs aswell as considering medicines optimisation issues.

Briony Leighton, Clinical Pharmacist (Arrow Health PCN), Primary Integrated Community Services Ltd Nottingham presented an SMR template toolkit she developed with an IT specialist to ensure a consistent and robust approach and support prescribing discussions & decisions with older people living in care homes.

Emma Davies, Advanced Pharmacist Practitioner in Pain Management, Cwm Taf Morgannwg University Health Board in Wales lead a practical session using case studies about dealing with chronic pain in structured medication reviews, following our update from Roger Knaggs in 2021 about the technical aspects of the new NICE guidance on Chronic Pain.

As he was a member of the DHSC short life working group, Steve presented about the latest Overprescribing report, in particular the need to tackle over prescribing at source i.e. repeat prescription systems.

Clare Howard, Medicines Optimisation Lead at Wessex AHSN joined us again to talk about the updated NHSBSA Polypharmacy Indicators and how they might help you better select the highest risk patients in your PCN who need an SMR.
Steve Williams discussed some interesting new research about clinician and patient/carer attitudes to Deprescribing.

We were very lucky to have Jonathan Underhill talking about the updated NICE guidance on shared decision-making as this is crucial for performing successful SMRs.
Jonathan has a wealth of knowledge in this field and he is a Medicines Consultant Clinical Advisor at the Medicines and Prescribing Centre at NICE and also the Medicines Optimisation Lead at the Keele Centre for Medicines Optimisation, School of Pharmacy at Keel University.

Dave Gerrard, NHS England Health Improvement Pharmacy Lead for Learning Disability and Autism, talked to us about how to utilise SMRs to support stopping over medication of people with a learning disability, autism or both.
We also heard from Mohammed Hassnawi, Practice Plus member and Clinical Pharmacist at Dorset Healthcare, who shared one of his recent SMR cases for learning.
